- Individualized examination consisting of specialized tests explicitly chosen for your condition
- Discussion of the findings
- A customized program aimed to treat your condition
What to bring with you on your first visit:
- Photo ID/ Insurance cards (if applicable)
- Doctor’s prescription if we do not already have it
- Attached “new patient” paperwork can be filled out by arriving 15 minutes before your appointment time
What to wear: Please wear comfortable, loose-fitting clothing and supportive shoes, as if you were going to the gym.
Your Commitment:
- We ask that you commit to your health and recovery by attending visits and following your home program.
- We understand that life happens, but please try to give a 24-hour notice for cancellations
- Finally, knowledge is power! Ask questions!
We strive to provide you with the highest quality and most up-to-date physical therapy treatments available.
