What to expect:

Your first visit will consist of a comprehensive evaluation performed by a licensed Physical or Occupational Therapist. This evaluation will include a:

• Review of your past medical history & current condition
• Individualized examination consisting of specialized tests explicitly chosen for your condition
• Discussion of the findings
• A customized program aimed to treat your condition

What to bring with you on your first visit:

• Photo ID/ Insurance cards (if applicable)
• Doctor’s prescription if we do not already have it
• Attached “new patient” paperwork can be filled out by arriving 15 minutes before your appointment time

What to wear: Please wear comfortable, loose-fitting clothing and supportive shoes, as if you were going to the gym.

Your Commitment:

• We ask that you commit to your health and recovery by attending visits and following your home program.
• We understand that life happens, but please try to give a 24-hour notice for cancellations
• Finally, knowledge is power! Ask questions!
