What Is Frozen Shoulder?

Unlike a rotator cuff tear, it is primarily characterized by stiffness and loss of motion rather than a specific tendon injury.

Frozen shoulder typically develops through three stages.

The Freezing Stage

During the freezing stage, shoulder pain gradually increases and movement becomes more difficult. Pain is often most noticeable at night and can last anywhere from a some weeks to several months.

The Frozen Stage

During the frozen stage, pain may begin to ease, but significant stiffness remains. Everyday activities such as reaching into a cabinet, getting dressed, or fastening a seatbelt can become difficult.

The Thawing Stage

During the thawing stage, shoulder movement gradually returns. Recovery can take months and, in some cases, considerably longer, with some cases lasting one to three years from start to finish.

The AAOS notes that frozen shoulder is more common among people aged 40 to 60. People with diabetes or thyroid conditions also have an increased risk, and Mayo Clinic points out that frozen shoulder can develop after the shoulder has been immobilized following an injury or surgery; women are affected more often than men.

The Key Difference in Symptoms

The most useful distinction between a rotator cuff tear and frozen shoulder is often the difference between weakness and stiffness.

With a rotator cuff tear, you may have difficulty lifting your arm because of pain or weakness. However, the shoulder may still have a relatively normal passive range of motion, meaning another person can move your arm farther than you can.

With frozen shoulder, both active and passive range of motion are restricted. In other words, you may not be able to move the shoulder yourself fully, and a physician cannot simply move it through its normal range either.

This difference is one of the main reasons a hands-on orthopaedic examination is so important for an accurate diagnosis.

How Are Rotator Cuff Tears and Frozen Shoulder Diagnosed?

Diagnosis begins with a medical history and physical examination.

Treatment for Rotator Cuff Tears and Frozen Shoulder

Rotator Cuff Treatment

Not every rotator cuff tear requires surgery. Depending on the injury, treatment may include activity modification, physical therapy, anti-inflammatory medication, injections, and other nonsurgical approaches.

Surgery may be considered when a tear causes persistent symptoms, significant weakness, or functional limitations that do not improve with appropriate nonsurgical treatment. The type of tear and the patient’s overall circumstances also have a role in determining whether surgery is appropriate. When surgery is recommended, arthroscopic rotator cuff repair may be used to reattach the damaged tendon to the upper arm bone.

Frozen Shoulder Treatment

Treatment for frozen shoulder generally focuses on controlling pain and slowly restoring shoulder motion.

Physical therapy is a central part of treatment. A physical therapist can build a stretching and range-of-motion program to improve mobility without unnecessarily aggravating the shoulder. Depending on symptoms, treatment may also include anti-inflammatory medication or a corticosteroid injection.

Surgery is not usually the first treatment for frozen shoulder. When symptoms remain severe and motion does not improve with nonsurgical care, procedures such as manipulation under anesthesia or arthroscopic capsular release may be considered.
