What is Thumb Basal Joint Arthritis?

Thumb basal joint arthritis, or carpometacarpal (CMC) joint arthritis, happens when the cartilage at the base of the thumb wears away. The CMC joint is a unique saddle-shaped joint that allows the thumb to pivot, rotate, and pinch against other fingers.

Because the CMC joint experiences high physical stress during pinching and gripping, the protective cartilage can degrade over time. When this happens, bone-on-bone contact occurs, causing inflammation, pain, bone spurs, and loss of joint stability.

Common Symptoms of Thumb Basal Joint Arthritis

Basal joint arthritis develops gradually, and its early signs are often mistaken for simple hand fatigue. Over time, however, the structural changes inside the joint become more pronounced.

Be sure to monitor for the following common symptoms:

  • Localized Pain: A sharp or burning pain at the base of your thumb, particularly during pinch activities (like opening jars, turning keys, or pulling zippers).
  • Swelling and Tenderness: Visible swelling, redness, and tenderness when pressure is applied to the joint at the base of your thumb.
  • Loss of Grip and Pinch Strength: Finding it increasingly difficult to firmly grasp objects or apply pressure with your thumb and index finger.
  • Limited Range of Motion: Stiffness in the thumb joint, making it hard to stretch the thumb wide or touch it to the tip of your pinky finger.
  • Physical Deformity: In advanced stages, a prominent bump may develop at the base of the thumb as bone spurs form and the joint shifts out of its proper alignment.

  • Suture Button Suspensionplasty: This procedure removes the arthritic bone (the trapezium) and uses high-strength fiber-wire and a small suture button to suspend the remaining thumb bone. This approach avoids harvesting large tendon grafts from the forearm, thereby reducing surgical trauma.
  • Arthroscopic CMC Debridement: For early-stage surgical candidates, a minimally invasive camera is used to clean out inflamed tissue and bone spurs, preserving the natural joint structure.

Dr. Panattoni is a board-certified, fellowship-trained orthopaedic hand surgeon at Vero Orthopaedics. He completed hand surgery and reconstructive microsurgery fellowships at the Christine M. Kleinert Institute for Hand and Microsurgery in Louisville, Kentucky, and the National University Hospital in Singapore.

A Fellow of the American College of Surgeons (FACS) with a Subspecialty Certificate in Surgery of the Hand, Dr. Panattoni has spent nearly a decade teaching surgical techniques. He offers his patients minimally invasive care focused on rapid recovery and long-term functionality.
